To be approved by PGN Logistics, all carriers must:
o One year of Active Authority
o Valid MC Number
o No Freight Guard reports containing hostage loads
o Active US DOT Number
o Signed Broker – Carrier Agreement
o Copy of carrier operating authority
o Signed W-9 Form
o Insurance certificate with contract specified limits
o Proof of Workers’ Compensation Coverage
o Satisfactory or Unrated Safety Rating Verification from: safer.fmcsa.dot.gov/CompanySnapshot.aspx
